Requirements to Register New Students
Students enrolling for Kindergarten must be five years of age by September 1, 2024 (EC 4800a); students must turn 4 years old by September 1, 2024 to enroll for Transitional Kindergarten (TK).
***For students who are 3 years old at the time of admission to TK (Education Code section 48000[c]), schools are recommended to initially apply the pre-kindergarten immunization requirements (PDF) and to follow up with these students once they turn 4 years old to apply the TK/K immunization requirements.
Please bring the following documentation to register:
1. Verification of Student Date of Birth:
✓ Original Birth Certificate, Passport, Certified Hospital Birth Record, or duly attested Baptismal Record.

Students Admitted at TK/K-12 Need: |
- Diphtheria, Tetanus, and Pertussis (DTaP, DTP, Tdap, or Td) — 5 doses
(4 doses OK if one was given on or after 4th birthday. 3 doses OK if one was given on or after 7th birthday.)
For 7th graders, at least 1 dose of pertussis-containing vaccine is required on or after 7th birthday. - Polio (OPV or IPV) — 4 doses
(3 doses OK if one was given on or after 4th birthday) - Hepatitis B — 3 doses
(not required for 7th grade entry) - Measles, Mumps, and Rubella (MMR) — 2 doses
(Both given on or after 1st birthday) - Varicella (Chickenpox) — 2 doses
3. Proof of Residency (ONE of the following is required) - Document must be dated within 30 days:
✓ Gas, Water, Electric, Telephone, rent receipt or a lease agreement. Bill must have parent’s name and date on the document. If unable to provide proof of residence, ask for an Affidavit of residence and return with proof of address of the person who signed the affidavit.
4. Parent Driver's License or other Picture ID: Be prepared to show a govermant issued ID.
5. Dental Examination / Oral Health Assessment (dated after 9/1/2023)
Note, dental examination should be dated no longer than 12 months prior to the first day of school and is only required for Kindergarten students.
6. Health Examination (CHDP): (dated after 3/1/2024)
Note, Health examination should be dated no longer than 6 months prior to the first day of school and is only required for Kindergarten students
7. Legal/Court Documents: (If applicable): Please provide the original current court documents. Copies will be made for your student's file. Custody issues can only be enforced by the school if the custody documents are on file with the school.
8. Middle/High School Students: Please provide a copy of the most recent report card or a transcripts (unofficial)
9. Special Education/504:
✓ If your child has an IEP (Individualized Education Program) or 504 plan, please submit a copy with registration.
